One thing they all have in common is banner advertising. Some of these are affiliate banners (eg Google Adsense), and others are actual paid banners. (eg American Muscle).
The banners are located in many different positions, from top of screen, to top of posts column, sidebars, etc.
Mostly, I have done this by manually editing the Wordpress theme files. Trust me. It's a pain.
Even worse, I have to do it all twice if there's a mobile theme in use as well.. :(
